Online Program Evaluation Form

Developed by the Staff Development Committee and used for Library Technology Training classes as well as Staff Development sponsored events, this form is an integral tool in helping us improve and develop our curriculum. Staff may use either the "traditional" paper copy or the new identical online version to comment on and evaluate these sessions. Your input is greatly valued and we hope that you will take the time to fill one of these forms out whenever you have attended a class or event. Both forms should be submitted to Laurie Power; the web form will be sent automatically, the paper form should be mailed.
