Library Staff Performance Evaluation Forms

This form has two parts: a self-evaluation to be completed by the employee and an annual evaluation to be completed by the supervisor. There are different versions of the self-evaluation form for exempt and non-exempt staff. The self-evaluation should be completed prior to discussion of the annual evaluation between the staff member and the supervisor.

Original copies of both parts of the signed evaluation forms should be submitted to the department head or division director who will submit them to the Library Personnel Officer by the end of March.
