Education will provice a plastic sleeve which is to be glued into the book. Change the pieces count for the book item record to 2.

*** CDROMs that come with kits: Some can just be put inside the books(teacher's editions usually) but some have more substantive manuals and need to stand-alone as a suppl. to a kit. For those can can be included with the teacher's edition, follow the guidelines for "CD-ROM w/book" above. For those that need to stand alone, follow guidelines for "CD-ROM standalone" above.
